Radiesse and Sculptra to treat naso-labial folds

Published: October 5, 2025
  • Radiesse is approved for correcting moderate-to-severe facial wrinkles and folds, including nasolabial folds.
  • It has shown good results for nasolabial fold correction: studies show that many patients have a visible reduction in depth of those folds at 3-6 months after Radiesse treatment.
  • Sculptra is also used in nasolabial folds, usually especially when the problem is not just the fold itself, but volume loss around the fold (cheek, midface decline) that makes the folds look more severe.

Comparison: Radiesse vs. Sculptra in Nasolabial Folds

FeatureRadiesseSculptra
Onset of improvementPretty much immediate (you can see filling of the fold right after treatment). Gradual. It may take weeks to months to see full effect, as collagen is stimulated and builds up.
Depth correctionGood for deeper folds — strong, firm filler like Radiesse can help lift and fill deeply. Works better when fold is part of more general volume loss, or when you want softer, more natural improvement over time. Might require multiple sessions.
LongevityTypically lasts around 12-18 months in folds. May diminish sooner in mobile, expressive areas. Can last 18-24 months or more, depending on how many treatments are done, how well collagen builds, and individual factors.
Number of sessionsUsually 1 treatment (maybe a small touch-up). Often multiple sessions (2-3) spaced weeks apart.
Risk / subtletiesBecause Radiesse is firmer and has larger particles (CaHA microspheres), there is more potential for visibility of the material, irregularity, or overcorrection if placed too superficially, especially in thin skin. Also, dynamic movement (smiling, talking) can affect how long it holds. Sculptra has risk of nodules or uneven texture if collagen response is uneven, or if injections are not well distributed. The changes are more gradual, which can be good for “softening” appearance rather than making folds disappear quickly.

Which One Might Suit You Better for Your Nasolabial Folds

Given what I’ve seen generally, here are suggestions depending on your priorities:

  • If your nasolabial folds are deep and you want them significantly reduced quickly, and you don’t mind a more obvious change right away → Radiesse would likely be better. It gives immediate lift / fill, good support, and more pronounced correction in one appointment.
  • If your folds are moderate, your skin is thinner, or you prefer a natural, gradual improvement (less “filler-look”), then Sculptra might be more appropriate. Over time, it will improve fold appearance by improving volume in the surrounding areas and making transitions look more natural.
  • If you have volume loss in cheeks or midface contributing to the appearance of the folds (not just the crease itself) → you might benefit from combining or focusing on those supporting areas (cheek lift / filling) along with or instead of filling the fold deeply. Sometimes working “around” the fold and lifting from above gives a more harmonious and longer-lasting result.
  • If you are concerned about possible visibility of filler, lumps, or overcorrection, choosing a skilled injector, possibly using a lighter touch or spreading product over a wider area rather than just in the fold, can make a big difference.
